There may indeed be one more step to take to make sure the name on your tax return matches the name registered with the Social Security Administration (SSA). Not addressing this might cause problems in the processing of your return or even a delay in a refund.
For people who are recently divorced and changed back to their previous last name, you need to notify the SSA of this name change by simply filing a Form SS-5, Application for a Social Security Card. There is no cost to do this and here are three ways to do it:
1. Visit your local SSA office. Massachusetts locations are listed online at
www.irs.gov/localcontacts/article/0,,id=98286,00.html. When you go to a Massachusetts office, you will need to bring a recently issued document as proof of your legal name change.
2. Complete the Form SS-5, use this link for a pdf,
